Switzerland Proof of Travel Form

For any member who did not sign in at our recent away match in Switzerland the proof of travel form is now available to download by clicking here: 2860.PROOF OF TRAVEL FORM.pdf.

For those new to the membership, we ask members to either sign in with FA staff in person where your membership card is scanned or details taken. For those that did not sign in, we ask that you submit a proof of travel form. Once received and accepted a cap can then be awarded. For members that did sign-in an away cap will be added over the next week.

We accept the following documents as proof of travel:

>- Original boarding pass
>- Original hotel receipt (We do not accept e-mail confirmation)
>- Original credit/debit card statement showing transaction location
>- Original mobile phone statement showing phone call location
>- Photograph identifying yourself at the match

Please note that the deadline to submit proof is 5pm on Monday 29 September 2014.

Please only attach documents which have your name printed on them. We will only accept one form per member; you must supply either a boarding card or a hotel receipt.
