The general principle is that you can't choose to give your ticket to someone else. However, where you are unable to attend the match for medical reasons, you are permitted to pass your ticket to a family member or friend. This is in the T&Cs. And it is fair to argue that unforeseen self-isolation is a medical reason.

But, I've never seen it explained by UEFA exactly how you go about passing your ticket to a friend in these circumstances. I suggest you contact UEFA asap and ask them how to do it. Explain your medical reason. Give them your friend's name and email address. They may even do the transfer to his phone for you. They might ask for medical evidence : a copy of a text message or email from an official source instructing you to isolate ?

Or you could follow Ian Panniers' suggestion which would cut out any problems with rules and officialdom. Note that your friend would also have to display his covid status on your phone.
